Universal Child Health Record

The State of NJ recently completed revisions to the Universal Child Health Record, endorsed by the American Academy of pediatrics, the NJ Academy of Family Physicians and the NJ Department of Health and Senior Services. All health care providers, child care providers and families are encouraged to use the standardized form to track immunications, medical conditions and preventive health screenings.
